To access the HealthConnect online system go to http://www.cmu.edu/health-services and select the HealthConnect button. You will need your Andrew ID and password to access the site. Note: If you do not take action by the deadline, you will automatically be charged for the basic Student Health Insurance plan. However, you will still need to enroll in the plan in order to receive your Student Health Insurance coverage. For students starting class in May, you will have two enrollment periods. You are required to take action prior to each enrollment period. Please read all information below. The student is REQUIRED to take one of the following actions for the first enrollment period May 1, 2012 - July 31, 2012:
Completing the enrollment form: The Enrollment form requests your address. Please do not enter your international address. If you do not have your Pittsburgh address yet, please leave this blank. The Enrollment form also asks for your 9-digit Carnegie Mellon Student Account #. If you reported your social security number in your application for admission, please enter your soc#. If you did not, you would have received an email from the office containing your initial password for your computer account. Please enter this 9 digit number that begins will 700. You are required to meet certain immunization requirements to be a fully matriculated student. The immunization requirement helps us to protect you and the community against vaccine-preventable diseases.
Requirements for All Full-time Students:
Two doses of measles (rubeola) mumps and rubella vaccine are required for all full-time students born in 1957 or later. If you had the disease as a child, you do not need the vaccine but do need to record the year that you contracted the disease on the form.
Due to a resurgence of pertussis among adolescents and young adults, we recommend (but do not require) that you also receive one dose of the tetanus, diphtheria and pertussis vaccine (Adacel or Tdap) before coming to campus.
You can request religious or medical exemption from immunizations by filling in the appropriate section on the online Health History Form.
Additional Requirements for International Students
Tuberculin skin test (PPD) 3-6 months prior to beginning classes. If the skin test is positive, results of a chest x-ray must be included. Please do not mail the x-ray films to our office. If you have had BCG, we will accept the results of an IGRA blood test.
Failure to complete Immunization Requirements
If you fail to fulfill the immunization requirements by the first day of classes of your first semester on campus, your enrollment status will change (administrative suspension) and a $50 late fee will be assessed to your student account.
Immunizations Form
Yes, you may receive immunizations prior to having access to the HealthConnect portal! We do not require your health professional to sign CMU forms. You will document your immunizations once you have your Andrew ID and access to the HealthConnect portal. Be sure to keep all documentation you receive when you receive your immunizations as reference in completing the online forms.

*If you are unable to complete the vaccination requirements prior to your arrival on campus, you will be able to obtain some vaccines through Student Health Services prior to the first day of classes.
If you should become ill during your tenure here, the health history information you enter assists us in providing you with appropriate care. You can request religious or medical exemption from immunizations by filling in the appropriate section on the Health History Form.
