In accordance with the Federal Immigration Reform and Control Act of 1986 (IRCA), you are required to provide Carnegie Mellon with documents establishing your identity and employment eligibility within three working days of your employment date. Under federal law, failure to comply will result in termination of employment. The following employment forms must be completed at payroll services.
- I-9 (employment eligibility) form

You can review the W-4 form and its instructions by downloading it from http://www.irs.gov/formspubs/lists/0,,id=97817,00.html - Direct Deposit form
- WTEX form (non-resident employee exemption certificate)
- Foreign National form (if applicable)

You must return a copy of your completed I-9 to the Office of Fiscal Management in Hamburg Hall (HbH) room 1107.
